To champion safety, we have a requirement that we fingerprint and background check everyone who volunteers at Midtown Church. We ask each individual 18+ years of age to complete a livescan fingerprint process under the organizational name of Midtown Covenant Church.
You must still complete this even if you have already done so under a current employer or other organization where you volunteer.
Capital Live Scan will no longer be providing forms.

and take with you to your visit.
If you have concerns or difficulty completing this, please talk with your Volunteer Team Lead for help.
